The 3 A.M. Playbook is educational material for licensed clinicians and clinicians in training. It is written for the moment before you walk into the room, and it assumes you are the one who decides what happens next.
It is not a protocol, not a guideline, and not a medical device. It gives no diagnosis or treatment for any individual patient and creates no clinician–patient relationship. Clinical responsibility stays entirely with the treating clinician.
Doses, thresholds and escalation rules in here reflect commonly cited adult practice and are attributed to their source where one exists. They vary between countries, between hospitals, and sometimes between wards. Where this pack and your local protocol disagree, your local protocol wins, every time. Read it before tonight, not during.
Part of this pack is written to be used with a large language model. The prompts come with their guard rails already in them, because the failure mode of these tools is agreement: ask a leading question and you will get a confident answer shaped like the one you wanted.
Treat anything a model gives you as a colleague's off-the-cuff opinion, not as a source: check it, and never paste identifiable patient information into a tool your institution has not approved.
This material is not written for patients or the public, and reading it will not help you manage your own care or someone else's. If you are worried about yourself or a person you're with, contact your own doctor or your local emergency service.
